Grant Requirements
All grant applications must be returned with the following to be presented for consideration:*
-Tax Return (Copies only)
Non-Profit Organizations
- Last Audited Financials
- Current Balance Sheet & Income Statement (Not older than 60 days)

*(Grant Packages MUST be sent all together and complete at the same time. Incomplete or piecemeal packages will NOT be reviewed)

- Diagnosis & Prognosis (if applicable)
- Estimate of cost
- Care Credit status (individuals must apply for care credit and submit evidence of either denial or approval with available balance)
- Telephone number that accepts calls from a "blocked' telephone number.

- Incomplete grant requests will only be held for 15 days
- Grant requests which have been approved are valid for 30 days only
