FAQs for Submitting A Grant Request
1. What information do I need to submit to be considered for my grant request?
A fully completed grant application including:
A. The Onyx & Breezy Foundation Questionnaire
B. Financials:
Individuals
-Tax Return
Non-Profit Organizations
- Last Audited Financials
- Current Balance Sheet & Income Statement (Not older than 60 days)
C. Estimate of Cost
D. Diagnosis & Prognosis
E. Care Credit status (for individuals only)
(Grant Packages MUST be sent all together and complete at the same time. Incomplete or piecemeal packages will NOT be reviewed)
2. What is Care Credit?
A healthcare financing plan developed to assist in veterinary care. You must have applied and been refused for partial or total costs to qualify for our assistance.
3. Where do I apply for Care Credit?
You can apply through your veterinarian or online. The response is usually immediate. Click Here to go to CareCredit
4. Can I get assistance with my previous veterinary debt?
No. We are not here to take the burden off of pet owners. We are here to see that your pet receives necessary medical treatment that is needed currently and cannot be covered by you at this time. Payments will be made directly to veterinarians, pharmacies, etc.
5. Can you tell me what type of assistance The Onyx & Breezy Foundation offers?
We do not pay for capital improvements to buildings or property. We will consider funding medical equipment, research, and medical treatment for animals where hardship is present, medication, pet food, spay and neuter, etc.
6. Why haven’t I received a response from The Onyx & Breezy Foundation?
You must provide us with a contact telephone number that accepts calls from a "blocked" number. If you cannot accept calls from a "blocked" number, we will not respond.
You haven’t submitted a fully completed application with all necessary documents. Due to the volume of grant requests we cannot take the time to help you complete your grant.
7. Can you help me find a pet or place a pet?
Unfortunately no. We do not have the capacity to shelter or foster animals. However, most communities do have a local shelter or rescues in their area. We would suggest you start there if you are looking to adopt. Check with local rescues if you are trying to place a pet rather than a shelter, unless it is clearly recognized as a NO KILL shelter.
