• Non-Profit Organizations
• Last Audited Financials
• Current Balance Sheet & Income Statement (Not older than 60 days)
• Diagnosis & Prognosis (if applicable)
• Estimate of cost
• Care Credit status (for individuals only)
• Please provide a contact telephone number that accepts calls from a "blocked' telephone number.

• Any incomplete grant requests will only be held for 30 days.
• Approved grant requests will only be valid for three months and then must be re-approved.
